Patient Tennessen Notice and Acknowledgment

The Medicinal Cannabis Patient Registry Program, operated by the Medicinal Cannabis Control Commission, collects private information from patients for enrollment. This information is used to communicate, verify eligibility, and administer the program. Patients must provide the required information, and its disclosure is limited to authorized individuals. Failure to provide the information results in denial of enrollment, hindering the purchase of medical cannabis. Patients can authorize caregivers to access their information, and law enforcement can do so with a warrant or court order.

Health Care Practitioner Acknowledgement

This is an agreement from a healthcare provider participating in the Medicinal Cannabis Patient Registry Program. The provider agrees to notify the Medicinal Cannabis Control Commission (MCCC) if their medical license becomes restricted, revoked, or if they stop caring for patients in the program. They confirm that they have diagnosed the patient with a qualifying medical condition(s) within their professional standards and have conducted an appropriate evaluation. If the patient is enrolled, the provider will continue treatment, report medical findings to the MCCC, and reissue a certificate every three years if needed. They also agree to comply with all program requirements set by the MCCC.
